What is Integrated Home & Day Care (IHDC)?

The Integrated Home & Day Care (IHDC) Programme is your one-stop solution for seamless, undisrupted, continued quality care.
The IHDC programme offers clients the option of having one provider managing all their care needs.
This will ensure smooth coordination, continuity of care and less administrative burden for clients.
Clients can be referred by a medical social worker or can directly approach Active Global to know more about the IHDC programme.
There are 3 levels of IHDC packages (tier 1 to tier 3), which correspond to different care needs. Your level will be determined after assessment by AIC.

What are the Services covered by IHDC?

Care Coordination and customised Care Plan designed by our multi-disciplinary team, including physician, physiotherapist, nurse, social worker, etc.

Home Based Care – Medical escort and transport, home personal care, home nursing, home therapy, meals, caregiver support and 24 hours helpline

Centre Based Care – assessments and regular reviews, day-care and rehabilitation services.

Tier 3 Package: $0.00 per month
