COPD Patients: Smoking Cessation advice, prevention of chest infections, medication prescription and compliance tracking, etc.

Diabetes Patients: Check Hba1c levels, work with nurses on medication prescription and compliance tracking, education on infection preventions, prevention of hype / hyper glycemia, etc.

Stroke patients: regular patient medical care as needed.

Home Medical Service Scope

Comprehensive care assessment and management of chronic, uncomplicated acute/sub-acute medical conditions
Prescription of appropriate acute and chronic medicines;
Educating the Client and caregiver on the Client’s medical conditions and the management plan;
Performing minor medical procedures such as simple wound debridement, intra-articular injections etc.
